Risk Assessment

I will meet with the client to understand the client’s perspective regarding present and future care needs.

A meeting will take place at the client’s home in order to provide and ensure a safe comfortable living environment, taking note of the following:

Assess how suitable the bathroom and bedroom facilities are for ongoing use, and identify high risk fall areas within, and immediately outside the residence.
I will make recommendations in order to safeguard the client, minimize the risk of falling, and will outline what needs to be put in place in order to achieve a safe environment within the property for the client.
A detailed Risk Assessment report will be compiled, which I will discuss in depth with the client in order to ensure the client is satisfied, and in agreement with my recommendations.
I will contact various suppliers and brief them on work to be done if necessary on behalf of the client.

Personal Care Plan

Providing an individual care and assistance plan for clients to ensure their specific mental and physical health conditions are supported and managed in the comfort of their own environment through managing the following:

The client’s current state of health.
Medical procedures that the client has undergone or any future medical procedures that the client may undergo.
Chronic and acute medical conditions.
Current medication prescribed by the client’s doctor.
Arrange with the client’s treating doctor and a registered pharmacy for the safe storage, and issue of medication should this be required by the client.
Identify facilities within the client’s property for a carer, in order to provide ‘live in’ care for the client, should this be necessary.
Advise on various Care options available regarding appropriate care, which can be tailored to meet the care needs of the client.
A detailed Care Plan will be compiled according to the client’s needs, which I will discuss with the client.

Secretarial Duties

Diary maintenance, scheduling and confirmation of various appointments, as well as keeping track of social engagements/medical appointments on behalf of the client.
Typing, printing and replying to emails, as instructed by the client.
Communicating with various suppliers under instruction from the client, and follow up regarding providers/services to be arranged timeously.
I will contact various suppliers and brief them on work to be done if necessary on behalf of the client.
Typing reports on behalf the client for client’s immediate family should this be required.
Making calls on behalf of the client if instructed to do so.
Organizing documents and implementing a filing system.
Maintaining and ordering of medication and Personal Care items.
Communicating with the client’s treating doctor regarding medical matters if instructed by the client to do so.
Communicating with the client’s pharmacy and pharmacist regarding medication if instructed by the client to do so.

Well Being

Brain stimulation and companionship – reading to the client, playing cards, doing the crossword puzzles, listening to music, speaking about current affairs, according to the client’s wishes and interests.
Exercise and speech therapy – under the guidance of a qualified practioner I am willing to assist with the client’s exercise programme, and speech therapy, as advised by the speech therapist.
Wardrobe care – making sure the client is well presented at all times, as well organising visits to the hairdresser, podiatrist and beautician, as required.

Medical Appointments

Driving and accompanying the client to various medical appointments, including hospital visits.
Liaising with the medical team if necessary regarding the client’s ongoing medical treatment, and prescription medication.
Collection of the client’s prescription and medication from the doctor’s surgery/pharmacy.
Diary management regarding medical appointments.

Nutrition

Social Occasions

Planning activities and excursions with the client according to the client's interests.
Driving and accompanying the client to various social engagements.
Day to day tasks, running errands on behalf of the client, e.g. post office, dry cleaners, bank, grocery shopping, etc.
